Location: Manhattan, Intake Date: 5/17/18, Intake Type: Owner Surrender
Medical Behavior: Blue, Sex: Male, Age: 9 years, Weight: 11 lbs
Original Location: 10463
DVM Intake Exam; Estimated age: ~9yrs based on o hx and PE. Microchip noted on Intake? scanned negative by LVT on intake. MC placed on intake. History : Surrendered by O due to housing issues and cost of medical care. O reports hx of seizures. Subjective / Observed Behavior – BAR, allowed all handling. Shaking, tense during exam. Objective ; BCS 6/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: dc 3/5; pd 3/5 ; PLN: No enlargements noted; H/L: 4/6 L systolic murmur ausculted; CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact male. testicles smooth and symmetrical ; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal. Assessment; hx of seizures, reported by O ; heart murmur; dental disease; overweight ; Prognosis: good. Plan: neurologic and cardiology work-up once placed / adopted ; daily dental care once placed / adopted. SURGERY: Permanent waiver due to heart murmur

Basic Information: Coco is a male white small breed dog that is estimated to be 9 Years old. His previous owner got him from a pet store 9 years ago and kept him until she was unable to care for Coco’s medical needs.

How is this dog around strangers? Coco is friendly with strangers and will approach them slowly until she becomes more comfortable.
How is this dog around children? Coco was not around children in her previous home.
How is this dog around other dogs? Coco was not around dogs in her previous home.
How is this dog around cats? Coco was not around cats in her previous home.
Resource guarding: Coco is not bothered by people going near her food or toys.
Bite history: Coco has no history of having bitten another person or animal.
Housetrained: Yes
Energy level/descriptors: Medium Energy, Mellow, Affectionate
Other Notes: Coco tends to bark and become very nervous around loud noises. She enjoys baths and being brushed for long periods of time. She likes to be picked up and carried around the house. She is not bothered by people trimming her nails.
Has this dog ever had any medical issues? Yes
Medical Notes: Coco has had seizures for the past few years.
For a New Family to Know: Coco is a mellow dog that enjoys following people around the house and being picked up and carried. She likes to play with balls and will play fetch in doors. She is used to being indoors and eats dry food. She is very house trained and rarely has accidents. She does not pull on the leash and enjoys slow walks.
Behavior Notes: Coco was very stiff bodied she had her tail tucked between her legs and was giving whale eyes. When i sat next to her she slowly approached me and started wagging her tail then jumped up for attention. She allowed me to pick her up carry her around and collar her.
