Objective: P = wnl R = wnl BCS 5/9 EENT: Mature cataracts OU, ears clean, no nasal or ocular discharge noted Oral Exam: moderate to severe dental tartar/periodontal disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: intact female, no MGT, no discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no menace OU (blind) Rectal: normal externally. IOP: 15 OS, 25 OD Assessment 1) Cataracts OU- blind 2) Dental Disease 3) Alopecia- r/o allergies (atopy v food) vs metabolic (cushings, hypothyroid) vs less likely parasitic vs less likely immune-mediated vs less likely neoplasia. Prognosis: fair Plan: Recommend NH placement- needs opthalmology consult and further workup for alopecia. SURGERY: Permanent waiver due to age
