DVM Intake Exam; Estimated age: Microchip noted on Intake? placed on intake ; History : SH; Subjective: BAR; Observed Behavior – Good on physical examination until added restraint was needed for an eye exam and vaccination and he tried to bite.Objective T = -; P = 120; R = -; BCS 3.75/9; EENT: Eyes moderate opacity of the lens secondary to bilateral cataracts, ears clean, no nasal or ocular discharge noted; Oral Exam: Missing teeth, severe dental disease include peridontitis and calculus ; PLN: No enlargements noted; H/L: NSR, NMA, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Two descended testicles ; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Assessment: severe peridontal disease; Prognosis: Good ; Plan: Tx peridontitis 1) clindamycin suspension 2 cc (50mg) BID for 7 days PO; Recommend – dental cleaning ; SURGERY: Permanent waiver due to age
