DVM Intake Exam; Estimated age: 9 years; Microchip noted on Intake? No; History : owner surrender;
Subjective: Observed Behavior – nervous; Objective ;P = 80bpm; R = wnl; BCS 7/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mild to moderate dental tartar, otherwise unremarkable; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact female, prominent vulva, no discharge; MSI: Ambulatory x 3 with toe-touching left hind limb lameness; (while sedated) positive cranial drawer noted when manipulating left stifle, rest of orthopedic exam wnl, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: formed stool, no masses noted; Assessment; 1) Left hindlimb lameness- top differential is a torn CrCl. Less likely but also possible would be a soft tissue injury vs micro-fracture. Other ddx that are less likely include tick-bourne disease, immune-mediated disease (IMPA), neoplasia. Prognosis: Fair; Plan: Took radiographs of left hind limb and pelvis; Administered Butorphanol 0.3 mg/kg IM once, Dexmedetomedine 10mcg/kg IM once, then 5mcg/kg IV as initial medications did not achieve adequate sedation. Left hind limb (including pelvis), AP and lateral interpretation- no overt fracture identified, no evidence of luxation noted. Left stifle appears mildly swollen. -Administered Carprofen 4.4mg/kg SQ once, will start 2.2mg/kg Carprofen BID starting tomorrow evening (4/12).- Consider surgical consult to further evaluate for possible cruciate injury as surgical intervention may be needed.SURGERY: Permanent waiver due to age (9 years)
