Medical Behavior: Blue; Sex: Spayed; Age: 13 years; Weight: 11 lbs Original

DVM Intake Exam: Estimated age: 13 Microchip noted on Intake? n Microchip Number (If Applicable): n History : OS. Subjective: QARH Observed Behavior – gentle allowed full examination. Objective:  T = – P = wnl R = wnl BCS 6/9  EENT:  OU – Cloudy, conjuctivitis, yellow mucoid discharge, painful to touch  Fluorscene Stain – NEGATIVE  Delayed Schirmer tear test – Dry  Ears clean, no nasal or ocular discharge noted Oral Exam: Moderate Dental disease PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic  ABD: Non painful, no masses palpated. U/G: Reported spayed, could not visualise an obviosu spay scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, matting CNS: Mentation appropriate – no signs of neurologic abnormalities  Rectal: externally normal Assessment: – KCS  – Moderate Dental disease – Moderate Diffuse Matting  Prognosis: good Plan: Complete intake   Optimmune BID for 14 days – THIS MEDICATION WILL LIKELY BE NEEDED TO BE USED LONGTERM IN THIS PATIENT  RECOMMEND REFERRAL. TO PRIVATE VETERINARIAN FOR FOLLOW UP EXAMINATION INCLUDING AN OCULAR EXAM WITH EYE PRESSURES

Basic Information:  Cici is a 13 year old, spayed, small mixed breed. Client got Cici from a friend 12 years ago and has had her ever since. Unfortunately, owner had to surrender due to having to go into a nursing home. She has no known medical issues and was last at the vet 5 years ago.

Previously lived with:  2 adults and 1 dog

How is this dog around strangers?  Cici is shy around strangers and will hide when they come over. She plays gently with adults.
How is this dog around children?  Cici has spent time around a 5 year old child and she was relaxed and respectful. She does not play with children.
How is this dog around other dogs?  Cici lived with her housemate, Minnie 26991, and they were relaxed, playful and respectful
How is this dog around cats?  Cici has not been around cats. Behavior is unknown.
Resource guarding:  Cici does not resource guard her food, treats or toys.
Bite history:  Cici has never bitten another animal or person.
Housetrained:  Yes
Energy level/descriptors:  low

Other Notes:  Cici is nervous during storms or fireworks. She isn’t bothered when being pushed off furniture, held or restrained, disturbed while sleeping, given a bath, getting brushed, or having nails trimmed and touched.

Has this dog ever had any medical issues?  No
Medical Notes;  No known medical issues upon intake.

For a New Family to Know;  Cici is described as being friendly, affectionate, and mellow. She has a low activity level and loves to eat and sleep. She will follow you around when you are home or stay in the same room. She doesn’t play with toys anymore. She is indoors only and eats human food. Her favorite treat is chicken. She is house trained and uses wee wee pads only. She is well behaved when left alone and and has never been crate trained. She doesn’t know any commands and has never been walked on leash.

Behavior Notes:  Upon intake, Cici hid under the chair but came to counselor when called in a soft voice. She allowed counselor to collar and pick up

Look:2. Dog pulls out of Assessor’s hands each time without settling during three repetitions.
Sensitivity:1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag:1. Dog follows at the end of the leash, body soft.
Paw squeeze 1:1. Dog gently pulls back his/her paw.
Paw squeeze 2:1. Dog gently pulls back his/her paw.
Toy:1. No interest.
Summary:Cici allowed all handling without issue.
