Medical Summary DVM Intake Exam Estimated age: 14 years Microchip noted on Intake? placed on LVT intake History :stray dog brought in by police Subjective: QAR Observed Behavior – suspect decreased hearing, sleeps very soundly, startles easily BCS 4/9 EENT: OU-mature cataracts no nasal discharge AU-mild waxy debris Oral Exam: mm pk, sl tacky; CRT <2 sec; severe dental disease, upper R canine very loose PLN: No enlargements noted H/L: grade V-VI/VI L systolic heart murmur with pronounced sinus arrhythmia; mild intermittent expiratory cough, lungs clear ABD: soft, nonpainful; soft stool palpable in colon; diarrhea staining around rectum U/G: male intact, 2 scrotal testicles MSI: Ambulatory x 4 with decreased weight bearing in hindlimbs; decreased hip extension bilaterally with discomfort; kyphosis of TL spine with pain on palpation of TL/LS CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Geriatric Heart murmur Severe dental disease Arthritis/back pain Diarrhea Plan Rec’d CBC/chem Rec’d cardiac work up for heart murmur metronidazole 50 mg/ml: 0.5 ml PO BID x 7 days rec’d pain management protocol based on blood work, if concerns with kidney/liver values then rec’d tramadol and/or gabapentin; if bw WNL, then rec’d NSAID trial Prognosis: guarded to fair SURGERY: perm waiver due to heart murmur
