Medical Summary DVM Intake Exam Estimated age: 8 -10y Microchip noted on Intake? Positive# AVD*103*309*309 History : stray Subjective: Observed Behavior – wags tail, allows all handling Evidence of Cruelty seen – neglect Objective P = 80 R = pant BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: severe dental disease PLN: smln enlarged H/L: HM 3/6 ABD: Non painful, no masses palpated U/G: no SS seen MSI: Ambulatory x 4, severe matting, no strangulation obvious CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: HM, rule out cardiomyopathy, ddz, matting Plan: shavedown. echocardiogram then +/- dental Prognosis: fair to guarded This breed is weel know for cardiomyopathy and should have a full cadiac workup before any anesthesia SURGERY: Permanent waiver due to age and heart murmur
