Observed Behavior -very sweet. Easily handleable. Did well for medical exam and procedures. Objective ; P =wnl; R =wnl; BCS=5/9; EENT: Eyes clear with nuclear sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam: adult dentition with severe dental disease and loose teeth, no oral lesions; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Distended and tense; U/G: MN; MSI: Ambulatory x 4, skin free of parasites, no masses noted, diffuse alopecia, lichenification, and erythema; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Diffuse alopecia and lichenification r/o Alopecia X vs endocrine vs allergies vs other; Geriatric; Severe dental disease; Elevated ALP/GGT; Prognosis: Good to fair; Plan: CTM while at BACC; CBC-nsf (platelet clumps noted) Chem-elevated ALP 297 (23 – 212 U/L), elevated GGT 14 (0-11), mildly elevated lipase 1,966 (200 – 1,800 U/L) T4 wnl; Rec dermatology consult; SURGERY: neutered
