I am a neutered male black chow chow mix. The shelter thinks I am about 1 YEAR 5 MONTHS old.
I came in the shelter as a OWNER SUR on 11/20/2015 from NY 11208, owner surrender reason stated was PERS PROB.

11/21/15 10:38 Basic Information Chulo is a 17 month old all black male neutered chow that came in as an owner surrender. He stated that he found him as a stray puppy and decided to keep him. He can no longer have him because he is currently homeless and doesn’t feel comfortable having Chulo sleeping on the streets. Chulo’s last vet visit was when he was last at our facility 5 months ago. However, he will undergo a complete medical examination. Social Life & Personality Chulo is respectful with children. Owner stated that he allows petting but does not solicit play. He has not interacted with other dogs and is playful and respectful with cats. Chulo is house trained and never has accidents. Chulo is kept indoors only and has a medium activity level. Owner describes him as playful, confident, excitable, affectionate, fearful and friendly. Behavior Issues Chulo has been to our facility in the past after biting someone. However he has not bitten or scratched another person or animal since then. He isn’t bothered with having his food bowl touched while eating or having objects or treats taken away from him. He barks when someone tries to trim his nails and with strangers and unfamiliar visitors. Upon intake, Chulo barked while in the lobby. However, he displayed no signs of behavior issues. He allowed his self to be collard with assistance from the owner. He was not photographed. For a New Family to Know Chulo knows the commands sit, lie down and stay. For exercise, owner walks him both on and off leash 4 to 5 times a day. When off leash, Chulo stays close to his owner’s side. He enjoys playing chase and tug with stuffed and squeaky toys. He is trained to sleep and eat in his crate and does well. When owner is home, he tends to follow him around and be in the same room as him. When left home alone he is well behaved. Chulo eats dry food only and sleeps in his crate.

Chulo came to us as an owner surrender on 11/20/15. It was reported by the owner that Chulo is respectful of children and cats as well. He does come to us with a previous history of biting someone out of fear during an argument. In addition the owner stated that Chulo is not bother when someone removes his food or toys. Chulo comes out of his kennel easily and walks well on the leash. When the assessor attempted to condcut the assessment Chulo began to spin and yelp uncontrollaby. While Chulo was in this situation he would not allow the assessor to touch him.This seems to be due to the medical condition of Chulo and for this reason the SAFER assessment was not conducted. When off leash with other dogs, Chulo is tolerant of the other dog but is more focused on trying to scratch himself.

scan positive: 981020013307551 neutered male weight: 52lbs bright eyes clean ears clean coat clean teeth nails down to kwick hot spots located underneath his tail and along the back of his hind legs as per 1067 start on 75mg of benadryl PO BID x 7 days along with genta spray topical BID x 7 days and place on vet check on last day to determine if e-collar should be removed overall body condition is good tense and nervous and fearful during exam – if provoked he will attempt to bite BARH NOSF
