08/16/2016 PET PROFILE MEMO
08/16/16 15:18 Basic Info: Chuki A1065547 is a 10yr old neutered male Chihuahua mix who was bought in a pet store in the Dominican Republic 10yrs ago and is being surrendered due to landlord issues, cost of medical care & no time. He was last taken to a veterinarian in 2014 and currently has a fatty mass on his throat. Socialization: Around strangers Chuki is friendly/outgoing or shy for a few minutes. He hasn’t been around children but had lived with a small dog briefly and was relaxed, affectionate & playful with them & also with the adults. Chuki hasn’t bitten/scratched any person/animal. Behavior: Chuki “goes” indoors at times and barks at noises especially the door bell. His behaviors are unknown for the following: having his food/food bowl, toys/treats touched/moved, having his nails trimmed or being held/restrained. Chuki isn’t bothered if someone unfamiliar approaches the home/family member but will bark. Chuki doesn’t mind baths but at times will try to get away. For A New Family To Know: Chuki is described as friendly, affectionate, mellow, playful (at times) with a medium activity level. When the owner is home he follows them around sometimes. Chuki enjoys squeaky & stuffed toys (at times). He’s a mostly indoor dog who’s walked 1-2x a day. He sleeps around the home and eats dry & wet Pedigree food 2x a day. When left home alone he isn’t destructive. Intake Behavior: Chuki seemed to have a loose body. He allowed all handling. Chuki allowed to be scanned (-), collared & taken pictures of. Chuki was then picked up/carried and placed into a kennel with ease.

08/18/2016 WEB MEMO
A volunteer writes: Chuki is so cute in his little doggy bed. Still, when he sees someone at his door, he springs out of his couch and is all ready to go out. Leash first, little guy. Chuki might be 10 years old, but he is still quite energetic. He is a little bit on the thin side but his coat is intact and well groomed. He seems to have a little cold, and a cherry tomato size mass (said to be possibly a lipoma) is hanging from his neck. A short walk around the front yard is to my little friend’s liking. He does not forget to do his business without marking (it is unknown if Chuki is fully housetrained). A stop by my lap is a must before returning Chuki to his kennel. Chuki was relinquished into our care because of housing issues, financial reasons and no time. He comes with very good recommendations, being welcoming of and friendly to all. Ten years of age is quite tough for a little guy like Chuki to find himself in a shelter environment when he had a family for so many years. Will you open your heart to this nugget and let him spend the rest of his life, safe, cared for and loved?

08/18/2016 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Chuki came to the care center as an owner surrender due to landlord issues. Owner reported that Chuki would bark at the door, enjoys squeaky toys and have lived with another dog. Upon intake, he had a loose body and allowed all handled. For his relief walk, Chuki did not pull on leash. Upon entering the assessment room, he was relaxed. His eyes were averted for Look and his body was neutral. He stood still for Sensitivity, body neutral. He followed slowly for Tag, due to his age, approaches at the end. He did not pull back his paw on either Squeeze attempt. He had minimal interest in Toy. When on leash with another dog at the care center, he did not approach. The Behavior Team recommends that, behaviorally, Chuki be placed in an Average home. Look: 1. Dog’s eyes are averted, body neutral. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still, eyes soft, body neutral. Tag: 1. Follows at end of leash slowly, approaches at end. Squeeze 1/2: 1. Dog does not respond at all for three seconds. Toy 1: Minimal interest. Dog sniffs toy. Dog to Dog: When on leash with another dog at the care center, he did not approach.

08/18/2016 GROUP BEHAVIOR EVALUATION – AVERAGE
Exam Type GROUP BEHAVIOR
When introduced off leash to other dogs in the care center, Chuki approaches and greets all with a wagging tail.
