DVM Intake Exam; Estimated age: 9yrs based on O hx; consistent with exam. Microchip noted on Intake? scanned negative by LVT on intake. History : Surrendered by O; unable to take P to new home. Came in with housemate 25278 Sasha. Subjective / Observed Behavior – BAR, very friendly and allowed all handling. Objective ;BCS 7/9; EENT: Nuclear sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam: dc 2/5; pd 2/5; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic — difficult to assess through panting; ABD: Non painful, no masses palpated; U/G: spayed female, scar present. vulva normal. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal on visual exam. Assessment; overweight; dental calculus; periodontal disease; senior; Prognosis: good. Plan: ok for adoption! weight management — lose ~5-7lbs. SURGERY: already spayed.

Basic Information: Chloe is a 9 year old spayed large breed dog that the owner got from a friend when Chloe was 4 months old. The owner is surrendering due to issues with housing. Chloe’s last vet visit was a few months ago for vaccines. Chloe does not have any injuries or health problems that the owner knows about.
Previously lived with: adults and 1 dog
How is this dog around strangers? Chloe is very friendly around strangers.
How is this dog around children? Chloe is very relaxed and playful with children, she plays gently with them.
How is this dog around other dogs? Chloe has lived with one dog her whole life and she is very relaxed and playful with the other dog.
How is this dog around cats? Chloe has never been around cats.
Resource guarding: Chloe is not bothered when her food bowl is touched while she eats or if a treat or toy is taken from her.
Bite history: Chloe has never bitten or scratched another animal or person.
Housetrained: Yes

Other Notes: Chloe is not bothered during loud noises, when she is taken off furniture, when she is restrained, disturbed while she sleeps, or when she is brushed. Chloe is fearful when she is bathed and the owner has never trimmed Chloe’s nails on her own. Chloe barks when the doorbell rings.

For a New Family to Know: Chloe is described as friendly and affectionate. The owner’s favorite thing about Chloe is that she is such a gentle giant and is very sweet. Chloe’s favorite thing to do is sleep all day. When the owner is home, Chloe stays in the same room as her. Chloe likes to play with bones. Chloe has been kept mostly indoors. Chloe sleeps anywhere in the home. Chloe is fed Pedigree wet and dry food mixed together, once a day. When taken outside, Chloe uses any surface. Chloe knows how to sit on command. On a leash, Chloe pulls very hard.
Behavior Notes: Upon intake, Chloe had a loose body and was circling around the Counselor to get petted. Chloe was wagging her tail and receptive to being spoken to softly. Chloe allowed all handling.

Look: 1. Dog’s eyes are averted, with tail wagging and ears back. Allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth.
Tag: 1. Dog follows at the end of the leash, body soft.
Paw squeeze 1: 1. Dog does not respond at all for three seconds. Eyes are averted and ears are relaxed or back.
Paw squeeze 2: 1. Dog does not respond at all for three seconds. Eyes are averted and ears are relaxed or back.
Toy:1. Minimal interest in toy, dog may smell or lick, then turns away.
Summary:Chloe approached the assessor with a soft body. She was social throughout, allowed all handling, and displayed no concerning behaviors.
Summary (1):4/15: When introduced off leash to dogs, Chloe is initially tense when approached. She relaxes to more neutral posturing after exchanging greeting.
