Medical Summary Sex: Female, intact Scan: Negative Estimated age: 6 years, based on teeth and demeanor Fleas: No flea dirt noted. S: Alert, allows all handling O: BAR-H, BCS 5/9, MMs pink and moist EENT: Dry black crusty discharge stuck in fur at medial canthus OU, pink alopecic ring around both eyes. No discharge AU, nose. Moderate tartar and gingivitis. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, slight tensing of abd when cranial abdomen is palpated deeply, no masses palpated M/S/I: Amb x4. No lesions noted. UG: Female, no spay scar palpated, nipples slightly pendulous, vulva slightly enlarged A: 1. Dental disease 2. Pink alopecic ring around both eyes – R/O mild environmental or food allergies Short-term prognosis: Excellent P: 1. Gave 1.4 ml pyrantel and Activyl 4-14 during exam 2. Long-term health care should focus on dental health and monitoring for further development of allergies 1088
