DVM Intake Exam; Estimated age: 12yo; Microchip noted on Intake? neg; History : stray. p has had abdominal massess for over a year. Subjective: BAR. Sweet and friendly, well behaved during PE and med handling. Observed Behavior – Objective T = P = wnl; R = wnl; BCS 5/9; EENT: Eyes clear, no nasal or ocular discharge noted. Ears mild black ceruminous debris and thickened skin. Oral Exam: mild to mod dental disease; PLN: No enlargements noted; H/L: Grade III/VI HM, NSR. CRT < 2, Lungs clear, eupnic; ABD: 2 very large bilateral inguinal hernias (R>L). Masses are semi firm, compressible, consistency of intestines/fat, and subjectively reducible to a certain degree. No pain or discomfort on palpation. U/G: FI, no MGTs, no vulvar discharge; MSI: Ambulatory x 4, skin free of parasites, no masses noted. Fair hair coat, patchy alopecia with scaling and crusting along dorsrum; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal rectal exam; Assessment: bilateral inguinal hernias; HM; dorsal dermatitis ; geriatric; intact female; Prognosis: fair, depending on if hernias can be surgically repaired, p is not an ideal surgical candidate given her age and heart murmur. Plan: – monitor in Medical; – cbc/chem; – 2 view AXR; – 2 view CXR; – Convenia 0.5mL SQ for dermatitis; – Miralax 1/8th teaspoon in food BID x 10 days. Extend and modify dose PRN; – Recommend transfer to specialty center for surgery and cardiology consult, and close anesthetic monitoring if surgery is pursued. ; – NH has been notified; SURGERY: Permanent waiver due to HM and age
