01/25/2017 RE-EXAM (LAST MAJOR EXAM)
Medical rating 3 NC – MAJOR CONDITIONS NOT CONTAGIOUS,
1/25/17 Hx: Very fearful of leash and human touch – lifts lips, growls, yelps and jumps forward to snap. Rads and bloods done – both unremarkable. Pt was put on Rimadyl and buprenorphine injections but pt cannot be restrained for meds. Treatment technician reports that pt has been eating the Rimadyl and is allowing her to get closer than before treatment – unclear if that is due to desensitization to shelter or to decreased pain. S: Yelping, bares teeth and snaps at all touch, even Assess-A-Hand when it is held 2-3 feet away from pt’s face. Could not do hands-on exam. O: BAR, appears to be underweight. EENT: Fur in front of eyes, difficult to evaluate. Teeth appear to be clean. PLNs: Not evaluated. H/L: No resp distress. Not ausculted. Abd: Not palpated. M/S/I: Stands with weight shifted off hind legs and onto front legs. Range of motion in neck appears to be wide but pt sometimes shrieks and snaps (jumping towards examiner) instead of tracking an object to L or R. Appears to have thoracic kyphosis and cervical lordosis. Shrieks when leash touches his neck or is moved near him. Walks well, no ataxia or lameness noted. Matted tail, fur on feet are urine-stained. A: 1. Shrieks, yelps and snaps at approaching objects – R/O chronic pain vs. extreme fear? 2. Matted, dirty fur coat Short-term prognosis: Fair P: 1. Stop buprenorphine as it cannot be safely administered. 2. Recommend placement with the ability to do long-term retraining, also consider gabapentin trial at 10 mg/kg (80 mg). 1088
