DVM Intake Exam; Estimated age: ~10yrs; Microchip noted on Intake? no; History : Owner surrender – reported liver issues, no paperwork provided; Subjective: BARH; Observed Behavior – Active, attention seeking, allowed handling, timid at times; Objective ; P = WNL; R = WNL; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Mild dental tartar, pink mm; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Neutered; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric, healthy — owner reported ‘liver issues’; Prognosis: Excellent; Plan: Continue to monitor while at BACC. Scheduled CBC/Chem
Progress exam:
History : Owner surrender 4/5- reported liver issues, no paperwork provided; 4/6-CBC: All results within normal limits; CHEM: Mildy elevated ALT 208 U/L (10-125); Subjective: BAR. Sneezing with nasal d/c. No cvd. Was moved into iso last week and never started on meds. Will start on meds today. Objective: EENT: Eyes clear, no ocular discharge noted, mild serous nasal d/c; H/L: Normal RR/RE, eupneic, sneezing; MSI: Ambulatory x 4, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; A: Geriatric; CIRDC; Mild elevation in ALT – R/O mild hepatobiliary disease vs other; Prognosis: Good Plan: Continue to monitor while at BACC. Start doxycycline 10mg/kg PO SID x14d until 5/1; Recheck day 7

Basic Information: Chico is a 10 year old altered male, black and brown small mixed breed. He came to his home in august 2017, from a previous owner and was surrendered due to sowing behavior concerns.

How is this dog around strangers?Around starngers Chico will bark at first but allows them to approach and pet him.
How is this dog around children?Chico has lived with a 6 and 11 year old child. Around the Children is playful and gentle.
How is this dog around other dogs?Chico has been around a small dog and is playful and gentle.
How is this dog around cats?Chico has never been around cats.
Resource guarding:Chico isn’t bothered when some touches his food bowl or toys. Chico isn’t bothered during a bath. Unfamiliar approaches the home he barks and growls.
Bite history:Chico bitten once recently.

Other Notes:Chico is house trained ad uses the potty on cement or grass. When left alone in the home alone chico is well- behaved.

For a New Family to Know; Chico is described as friendly, and shy. Chico likes to play with balls and sqeasky toys. He is feed wet and dry food, of the wellness brand and is fed twice a day. Chico has been crate trianed and known commands sit, stay and roll. Chico goes on brisk walks on leash .
Behavior Notes:Upon intake Chico was friendly and approached the counselor and solicited petting. he was collared and a photo was taken.

Look:2. Dog pulls out of Assessor’s hands each time without settling during three repetitions.
Sensitivity:1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag:1. Dog follows at the end of the leash, body soft.
Paw squeeze 1:1. Dog gently pulls back his/her paw.
Paw squeeze 2:1. Dog gently pulls back his/her paw.
Toy:1. Dog settles close, keeps a firm grip and is loose and wiggly. Dog does not place his/her body between you and the toy.

ENERGY LEVEL:His previous owner describes Chico as a playful, gentle dog. Chico displayed medium energy and movement throughout assessment, and will need daily mental and physical activity to keep him engaged and exercised.

Recommendations comments:No children: Chico showed no issues in the care center, he has shown a warning and distance-increasing behaviors to different handling triggers in the home. Due to these behaviors we recommend an experienced adopter and an adult-only home.
Bite history:Bite Details: The owner’s 11 year old son attempted to take Chico out of his crate. Chico rushed forward, biting him on his left hand, leaving two small wounds.
