KNOWN HISTORY: None 5/17/17 Unaltered Female, Stray Other notes: Upon intake Checkers was shy but allowed all handling. SAFER ASSESSMENT: 5/19/17 Look:1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity:1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag:1. Follows at end of leash, body soft. Squeeze 1& 2: 1. Dog gently pulls back her paw. Toy: 1. Minimal interest in toy, dog smells, then turns away. Summary: Checkers was friendly and social towards the assessor. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: When off leash with dogs at the Care Center, Checkers solicits and engages in gentle play in a group of playful male and female dogs. Checkers was surrendered as a stray so her past behavior around dogs is unknown. The Behavior Department recommends that Checkers be placed with resident dogs that match her gentle play style. MEDICAL BEHAVIOR: 5/17/17: During her medical examination, Checkers was shy but very sweet and allowed all handling. ENERGY LEVEL: We have no history on Checkers so we cannot be certain of her behavior in a home environment. However, she is a young, enthusiastic, social dog who will need daily mental and physical activity to keep her engaged and exercised. We recommend long-lasting chews, food puzzles, and hide-and-seek games, in additional to physical exercise, to positively direct her energy and enthusiasm. When off leash with dogs at the Care Center, Checkers solicits and engages in gentle play in a group of playful male and female dogs. Checkers was surrendered as a stray so her past behavior around dogs is unknown. The Behavior Department recommends that Checkers be placed with resident dogs that match her gentle play style. 5/18: Checkers greets a novel dog displays soft and nervous body language. She slowly approaches to greet him with a low wagging tail and lowered body. She is initially nervous of the higher energy play, but is able to solicit and engage in gentler play. She becomes more relaxed as the session continues. 5/19-5/24: Checkers solicits and engages in gentle play in a group of playful male and female dogs.

05/17/17 DVM Intake Exam Estimated age: 8 months Microchip noted on Intake? No. History : Stray dog found on a roof, noted to have scratches on her head. Subjective: Observed Behavior – Shy, but very sweet. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=92, RR=16. BCS=3/9. ORAL: All adult teeth erupted, no oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. No coughing. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, no spay scar. MS: Ambulatory x 4, no lameness INTEG: Erythema and superficial scratches on dorsum of head, no bleeding or discharge. Erythema, abrasions on ventral thorax and axillae. Erythema and multiple papules, pustules on ventral abdomen. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx. 8 month old puppy, underweight, with superficial pyoderma. P: Rx Cephalexin 500 mg 1 PO BID x 14 days. TID feedings. Temporary waiver from OVH surgery due to underweight condition and pyoderma in area of surgical incision. Rec. foster or NH placement to help socialize. Excellent prognosis.
