PE: EENT NUCLEAR SCLEROSIS OU, MILD WAXY DEBRIS IN BOTH EARS, PLN WNL, ORAL EXAM MODERATE DENTAL TARTAR/GINGIVITIS, THORACIC AUSC WNL, ABD PALP WNL, M/S WNL, UROGENITAL WNL (INTACT MALE), SKIN/HAIR — INFLAMED NAIL BEDS ON ALL FOUR FEET — BRITTLE ENLARGED NAIL GROWTHS ON ALL FOUR FEET, ULCERATION OBSERVED ON NASAL PLANUM AND PAW PADS. MILD SWELLING/EDEMA IN BOTH HINDLIMBS AT HOCK REGIONS. TREATMENT: PROVIDED 2.5ML PPG IM AND 1ML BUPRENEX IM AT TIME OF EXAM. PAWS PADS AND NAIL BEDS CLEANED WITH CHLORHEXADINE AND TAB OINTMENT APPLIED TO ULCERATED AREAS. PLAN: PROVIDING 250MG CLAVAMOX PO BID FOR 7 DAYS AND 50MG TRAMADOL BID FOR 7 DAYS. RECOMMEND COMPLETE WORKUP WITH LOCAL VETERINARIAN FOR SUSPECT PEMPHIGOUS FOLIACEOUS/VULGARI OR OTHER AUTOIMMUNE SKIN DISORDER. IF NO PLACEMENT RECOMMEND EHR DUE TO UNDERYLING DISEASE PROCESS. PROGNOSIS IS FAIR — REQUIRES FURTHER DIAGNOSTIC WORKUP AND MEDICAL CARE
