Medical Summary DVM Intake Exam Estimated age:reported to be 2 years Microchip noted on Intake? neg History : owner surrender because he is growling at kids Subjective: BAR Observed Behavior – nervous; growls when approached; lunged and snapped when trying to muzzle Objective BCS 5/9 EENT: AU-lichenification/crusting of pinna, appears to extend down into canals; did not perform thorough aural exam due to temperament thickening/crusting of nasal planum Oral Exam: did not perform due to temperament PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: did not palpate due to temperament U/G: male intact; 2 normal scrotal testicles MSI: ambulatory x 4 with no apparent orthopedic disease chronic dermatitis on face, abdomen extending inguinally down medial aspect of hindlimbs-lichenification, crusting, erythema, alopecia and dorsal aspect of digits on all paws (worse on hind) CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Dermatitis-r/o chronic atopy vs demodex vs other Plan skin scrape-pinna, dorsal aspect of digit IV on RH; tried to scrape forehead but p would not allow-negative simpicef x 21 days rec’d treatment for demodex with nexgard or bravecto despite neg skin scrape rec’d derm consult Prognosis: good SURGERY: temp waiver due to dermatitis
