Medical Summary Estimated age: 9 yrs Microchip noted on Intake? No. Stray dog, no medical concerns noted. Fearful, trembling, allows all handling. BARH. mm=pink, moist, CRT<2 s. HR=136, RR=20. BCS=5/9. ORAL: Moderate to heavy calculus, gingivitis. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted H/L: Lungs clear. Gr. 3/6 left sided systolic murmur. No coughing, no dyspnea. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, no spay scar found. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Mild dandruff. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx 9 yr old dog with cardiac murmur, dental disease. Murmur – mitral valve disease. P: Permanent waiver from OVH surgery due to age and heart murmur. Rec. cardiac ultrasound when placed. Good prognosis with appropriate management.
