LVT intake; Spayed female ; Scan positive for a microchip ; 7 years old according to Chameleon ; OU=clear; Nose= No discharge ; AU=clean–did not like them touched. Known to have ear infectinons; Teeth have staining with mild tarter; Coat is clean ; Overweight; Poss. UTI–urine with blood seen in carrier and cage.–Also, reported while she was here in August; Dewormed with Pyrantel; Flea Treatment= Para Defense; As of now no signs of CIRDC; As of now no V/S/C/D

[DVM Intake] Estimated age: 7 years; History : stray; hematuria seen by two staff members; Subjective: Observed Behavior – tense but able to examine with muzzle; Objective; BCS= 8/9; EENT: Eyes clear, AS- erythema and moderate brown debris, AD- clean, no nasal discharge noted; Oral Exam: muzzled, no oral exam; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: FS, tattoo seen, no vaginal discharge; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment; otitis externa AS; overweight; hematuria- R/O UTI vs urolithiasis vs neoplasia vs other; Prognosis: good 3 NC; Plan: Continue clavamox; applied osnuria AS, repeat in 1 week
