Medical Summary BAR on presentation. Patient was nervous, however allowed full exam. BCS: 5/9. EENT: NS OU, ears clean- significant amount of hair within canals AU, no nasal discharge noted. Oral Exam: Grade II/IV periodontal disease. PLN: No enlargements noted. H/L: NSR, Grade III-IV/VI holosystolic heart murmur ausculted. CRT < 2, Lungs clear, eupnic. ABD: soft, non-painful, no masses palpated. U/G: neutered male. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities. Rectal: not examined. Assessment: 11 yo MN Poodle mix with: Grade III/VI HM – r/o MVD vs. CHF vs. other, Dental disease. Plan : Murmur requires further diagnostics at full service clinic/cardiologist (ideally Blood work, chest x-rays, blood pressure, ECG) upon placement. Prognosis is Fair upon diagnosis and appropriate treatment
PROFILE :
Charlie is an 11 year old neutered Black and white Poodle mix; he was surrendered due to the children in the home developing allergies. He has no known health issues or injures and has not been to the vet recently.
Around strangers Charlie is shy at first and later becomes friendly. He has been around a 6 month old and a 1 year old, he is relaxed and gentle with both children. Around other dogs Charlie is respectful but mostly keeps to himself. Charlie has no bite history.
During storms and fireworks Charlie is nervous and runs to his owner. He is not bothered when someone touches his food bowl. Charlie needs some time to get to know the individual before they try to pick him up. He is very tense during bath time, but allows the handling. When an unfamiliar person Approaches the home or Charlie’s family he barks at first and later becomes uninterested.
Charlie was described as shy, Affectionate and playful with a medium activity level by his Previous owners. His favorite thing to do is to sleep and cuddle with his owner, and he is bilingual in Spanish. Charlie likes to play with ball toys and is feed Purina one dry food, twice a day. Charlie is housetrained and uses a wee-wee pad and goes outside on the grass twice a day. He is well-behaved inside and outside. Charlie has never been crate trained and goes on walk on leash and off-leash and stays close to the owner.
Upon intake Charlie was very timid and snapped at the counselor when they approached him. Charlie was scanned and collar buy owner .
