03/08/2016 PET PROFILE MEMO
3/8/16 17:32 Basic information: Charlie is a roughly5 yr. old Chihuahua Pug mix. The Owner got her from a pet store; he has owned Charlie for over 4 yrs. The owner surrendered Charlie because of a conflict with the owner’s child. Charlie has been limping on his back right leg for an unknown reason; he has never been treated and has never been taken to a vet. Socialization: Around strangers Charlie is very shy and fearful. Charlie has not lived with children between 2 and 17 year, he is very fearful and does not like to be near children. Charlie has never been around other animals so the owner has no idea how he would behave around them. Charlie has bitten one of the owner’s children because the child would always put his face in front of him and Charlie does not like to be snuggled or handled by children. Behavior: The owner stats that Charlie is not bothered when some moves his food or treats while he is eating, when someone takes he is toys away or when someone disturbs him while sleeping. Charlie likes to be brushed and given bathes but has never had his nails trimmed. When an unfamiliar person or animal approaches a family member or his home he Barks and growls at the person. The says that Charlie is terrified of loud noises such as storms or fireworks and will hide or panic around the house until the noise stops. For a new family to know: The owner describes Charlie as Shy and fearfulwith a medium activity level. His favorite activity is to play fetch with his owner. In the home Charlie tends to follow the owner around the house and is kept mainly indoors. Charlie sleeps in the floor of the bedroom on a dog bed. Charlie was fed Pedigree dry dog food three times a day. He is house trained and has rarely has accidents in the home (he may poo or pee in the house if not walked), when outside he goes to the bathroom on or around trees and grass. Charlie knows the commands “sit “and “stay”. For exercise Charlie is used to brisk walks on the leash and once a day and pulls lightly on the leash. He is allowed to play off the leash in the dog park sometimes, he wanders off some to explore the surrounding are but comes back when called when off the leash. Behavior during intake: Charlie was somewhat tense with stiff body, when approached by staff Charlie tried to hide from the staff member but allowed slow approach, owner was able to assist us, scanned (-), collared and placing in dog kennel.

03/10/2016 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Charlie came into our center as an owner surrender after reportedly biting a family member (small child) on the mouth and nose. It is reported Charlie is shy and fearful around strangers and does not like to be near children. Behavior around other animals is unknown. Owner reports Charlie is not bothered if someone touches his Food, Toys, Treats. It is reported he will bark and growl if someone unfamiliar approaches the home. Owner reports he is housetrained to go outside on cement or grass, but may have accidents in the home if not walked regularly. During intake, Chalrie was somewhat tense with stiff body, trying to hide from staff whenever approached. He did not pull on leash for his relief walk. Charlie attempted to bite leash and collar while collaring to begin SAFER assessment, therefore all tactile portions of assessment were skipped. He showed minimal interest in Toy, though this may be due to environment as owner reports his favorite activity is to play fetch. He approached the helper dog confidently, ears forward, tail above spine level. Due to his bite history and attempts to bite during collaring, Charlie must be placed with a New Hope partner. He would benefit from a calm, quiet, structured environment with no sudden movements and loud noises. Toy: 1. Minimal interest in Toy, dog may smell and/or lick, then turns away. Dog-dog: 2. Dog approaches the helper dog, body soft, tail well above spine level, ears forward.
