Hello, my name is Charlie. My animal id is #11757. I am a male white dog at the Manhattan Animal Care Center. The shelter thinks I am about 9 years old.
I came into the shelter as a stray on 04-Nov-2017.

Oral Exam: severe periodontal disease PLN: submandibular LNs mildly enlarged, symmetrical H/L: NSR, NMA, CRT < 2. Increased BVS bilaterally, snorting/honking, eupnic ABD: Non painful, no masses palpated U/G: intact male, 2 testicles MSI: Ambulatory x 4, skin free of parasites. Multiple dermal masses : one round raised 2cm on left lateral thorax, and two round 1 cm raised masses on right dorsal and lateral thorax. Dirty haircoat. Diffuse hairmatting, especially on muzzle, limbs, and caudal dorsum. Hypotrichosis along dorsum. Multiple blackheads. Overgrown nails. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: normal externally. Several episodes of vomiting reported S/O -BAR, allows handling but appears somewhat anxious -mm pk/pigmented, tacky; difficult to assess CRT; severe dental disease -slightly enlarged/prominent L submandibular LN, likely secondary to dental disease -panting heavily, laryngeal stertor -referred upper airway noise, panting heavily but normal effort -severely distended/pot bellied abdomen; slightly tense on palpation; no obvious fluid wave -dried diarrhea in fur around rectum A 1. Laryngeal stertor-suspect secondary to elongated soft palate 2. Heavy panting-concern for possible pulmonary thromboembolism; r/o pain/discomfort secondary to nausea, abdominal pain, etc 3. PU/PD, distended abdomen-suspect cushing’s disease 4. Vomiting, possible diarrhea-r/o pancreatitis vs other 5. Dental disease P Gave 0.3 ml torb SQ-seemed to make him somewhat calmer but was still intermittently panting Gabapentin 250 mg/5 ml: 1 ml PO BID, first dose given at 4:30 pm Cerenia 0.7 ml SQ SID x 3 days Chest radiographs tomorrow Monitor breathing, SPO2 Recommend low dose dexamethasone test after placement to confirm suspected cushings +/- abdominal ultrasound

Upon intake, Charlie was very excited and wagging his tail. Charlie allowed all handling.
Basic Information:: Charlie is a 9 year old unaltered small breed dog that the owner inherited last year after his grandmother passed away. The owner cannot have Charlie due to financial issues. The owner has never taken Charlie to the vet. The owner says that sometimes Charlie has breathing issues.
Previously lived with:: 2 adults
How is this dog around strangers?: Charlie is cautious around strangers but then he warms up and begins wagging his tail, approaching them slowly.
How is this dog around children?: Charlie has spent time around a number of children, he is relaxed and playful with them. Charlie plays gently with children.
How is this dog around other dogs?: Charlie is relaxed and playful with the female small breed dog at home.
How is this dog around cats?: Charlie has never been around cats.
Resource guarding:: Charlie is not bothered when his food bowl is touched while he eats but he growls when a treat or toy is taken from him.
Bite history:: Charlie has never bitten or scratched another animal or a person.
Housetrained:: Yes
Energy level/descriptors:: medium
Other Notes:: Charlie hides when a fire truck passes by outside. Charlie is not bothered when he is taken off of furniture, when he is restrained, when he is disturbed while he sleeps, when he is brushed, or when he is bathed. Charlie is afraid when his nails are trimmed. Charlie barks when the doorbell rings.
For a New Family to Know: The owner describes Charlie as very shy and affectionate. The owner’s favorite thing about Charlie is how good he is with kids. Charlie’s favorite thing to do is play with his stuffed toys. When the owner is home, Charlie follows him around. Charlie likes to play with stuffed toys and he enjoys playing fetch and tug. Charlie has been kept indoors only. Charlie sleeps under the bed. Charlie eats Pedigree wet and dry food, 1.5 cup of dry a day and if he is eating the wet food then 1/2 can twice a day. Charlie will use a wee-wee pad at home if one is put down for him. When taken outside, Charlie uses a hydrant. On a leash, Charlie pulls lightly.

Sensitivity:: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
