Medical Summary Estimated age: Microchip noted on Intake? Stray, deep, chronic ulveration of corneas. scarring and edema. minimal uptake of stain (althought TAB had already been applied). Appear totally blind. changes may be due to glaucoma but tonopen not available to test. Back is arched at rest (kyphosis) but not painful on palpation allows all exam, very calm P = 80 R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: hevay dental tartar PLN: SMLN enlarged H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities–probable IVDD Assessment: blind, chronic changes to eyes, possible glaucoma. Possible chronic IVDD Plan: rimadyl 12.5mg/day, tab ou bis needs IOP measured, possible enucleations Prognosis: fair Permanent waiver due to age
