DVM Intake Exam: Estimated age: 10-12yrs based on PE. Microchip noted on Intake? scanned negative by LVT on intake. MC placed on intake. History : Stray dog found walking on street. Subjective / Observed Behavior – none BCS 7/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: dc 3/5; pd 4/5 PLN: No enlargements noted H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic; referred upper airway noise; coughing when trachea pressed ABD: Non painful, no masses palpated; heave line palpated U/G: intact male. testicles are smooth and symmetrical MSI: Ambulatory x 4, skin free of parasites, no masses noted, bilaterally symmetrical alopecia along dorsolateral body CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: grossly normal Assessment: bilaterally symmetric alopecia r/o low T4, Cushings, low normal T4 — r/o sick euthyroid, hypothyroid, malnutrition, spurious, other non thyroidal illness dental disease overweight collapsing trachea geriatric Prognosis: good. Plan: CBC / Chem / T4 skin scrape — negative dental care once placed / adopted weight management borderline hypothyroid rec placement with ACTH stim test to rule out cushings. SURGERY: Permanent waiver due to age
