DVM Intake Exam; Estimated age: approx. 4 yo; Microchip noted on Intake? neg; History : Stray; Subjective: BAR. Severe hair coat matting. Dried blood on rear end. Tried to bite several times during PE, limited exam of face and front end. Observed Behavior – Objective T =; P = wnl; R = eup; BCS 5/9; EENT: Crusted discharge OU, ears unable to examine due to matting and behavior, no nasal discharge noted; Oral Exam: unable to assess due to behavior; PLN: No enlargements noted; H/L: Grade IV/VI left systolic HM. NSR, CRT unable to assess, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Intact male; MSI: Ambulatory x 4, no ectoparasites seen. Severe hair coat matting along ventrum, both ears, and all 4 limbs. Round firm pink non-irritated sarcoma-like mass on right side of rectum. Bleeding bruised area ventral to the left anal gland. ; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: moderately sized anal glands, did not try to express as pet would not allow; Assessment: Heart murmur; Severe hair coat matting, underlying dermatitis; External mass on rectum; Wound near left anal gland; Prognosis: fair; Plan: – Convenia 0.6mL SQ – Start Gabapentin 100mg PO BID – Sedated hair matt clipping and bath tomorrow: NPO overnight; Premed = torb 0.2mg/kg IV or IM; Place IVC; Induction = valium 0.5mg/kg IV and propofol IV to effect; Considering intubating and maintaining on Isoflurane during de-matting. – Post placement/adoption, consider biopsy or surgical removal of rectal mass, 3 view met check, and full blood work. SURGERY: Permanent waiver due to HM

Cereal is a little dog that came to ACC severely matted. After a quick shave down he feels much better! He has a mass that will require further veterinary work-up, as well as a heart murmur.

Basic Information:
Cereal is approximately a 4 year old male white and tan small mixed breed dog. Cereal was found as a stray on the night of 02/13/2018. Cereal was taken home for the night by the finder and was surrendered because of his medical condition.
How is this dog around strangers? Cereal was friendly with the finder and allowed the finder to pet him and take him home.
Bite history: Cereal did not try to bite the finder
Has this dog ever had any medical issues? Yes
Medical Notes: Cereal was found with an open bloody mass in his rear area.
Behavior Notes:
Upon intake, Cereal allowed a leash to be put on him and to be collared. Cereal allowed to be pet and allowed to be carried into his kennel.
