Medical Summary DVM Intake Exam Estimated age: 15 years Microchip noted on Intake? scan negative History : stray, was brought to a vet by the finder and was told that she has Cushings, drinks a lot of water per finder Subjective: Observed Behavior – attempted to bite during exam  Objective BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: limited oral exam, severe dental calc seen PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Tense, difficult to palpate U/G: F, unable to see spay scar MSI: Difficulty getting up, stiff in hindlimbs when walking, skin free of parasites, no masses noted, patches of alopecia on dorsum CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe dental disease alopecia- R/O endocrine disease vs atopy vs mites vs other history of polydipsia- R/O Cushings vs renal disease vs DM vs other difficulty rising, hindlimb lameness- R/O osteoarthritis vs other Plan scheduled bloodwork for 6/23 Prognosis: guarded SURGERY: Permanent waiver due to age
