DVM Intake Exam; Estimated age: 11; Microchip noted on Intake? n ; History : OS; Subjective: BARH; Observed Behavior – Gentle – easy to examine ; Objective ; T = -; P = 90; R = wnl ; BCS = 7/9; EENT: EARS SEVERE OTITIS EXTERNA – Stenosis, hyperkeratosis, lichenfication, erythema, +2 rods +2 cocci ;Could not visualize ear canal with otoscope due to stenosis, Eyes clear, ears clean, no nasal or ocular discharge noted,nose hyperkeratosi; Oral Exam: Severe dental disease, edentulous ; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Vulva UR – Intact ; MSI: Ambulatory x 4, skin free of parasites, no masses noted, Matting around feet ; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: externally normal ; Assessment – Chronis Otitis Externa ; Prognosis: Good ; Plan:  – Prepared 9ml saline, 15 ml Baytril 22.7 mg and 6ml Dexmethasome SP (4mg/ml) 3 ml both ears for 3 weeks – Rimadyl 2mg/kg PO WITH FOOD for 4 days (reduce otic inflammation) – Clip matting around feet RECOMMEND – Dental cleaning/brushing/dental treats and weight loss; Re-check examination for otitis externa in 2 weeks ; SURGERY: Permanent waiver due to age
