DVM Intake Exam; Estimated age:8-10y; Microchip noted on Intake?n; History :stray; Subjective:reluctant to stand. unsure if terrified and “shut down” or actually weak. will stand and walk if forced to; Observed Behavior -quiet, does not interact; Objective ; T =; P =60; R =wnl; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam:pink mm moderate tartar; PLN: No enlargements noted; H/L: NSR, 3/6 heart murmur, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G:2 testes; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: heart murmur. lethargy; Prognosis:fair; Plan: chest/pelvic rads; chest rads wnl. pelvic rad no interpretable (dog would not stay still) SURGERY: Permanent waiver due to age
