Objective P = wnl R = eupneic BCS 5/9 EENT: Eyes have nuclear sclerosis ou with tear staining but no active d/c, ears have moderate debris and stenosis of canal, no nasal discharge noted Oral Exam: adult dentition, severe tartar with oligodontia and prognathism, no oral lesions noted PLN: No enlargements noted H/L: NSR, Grade 3/6 HM, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, 2 testicles descended MSI: Ambulatory x 4, skin free of parasites, ~3cm round soft tissue mass on the dorsal lumbar region, healthy hair coat, tear and salivary staining on face CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Grade 3/6 HM Geriatric Mass on dorsal lumbar ~3cm Severe dental disease Plan: Continue to monitor while at BACC Rec cardiologist consult Rec biopsy of mass and histopathology Rec dental if bw/ua and heart ok Prognosis: fair SURGERY: permanent waiver due to heart murmur and age

Upon Intake Cappucino was relaxed and friendly he allowed the counelor to pet him an dcollar him with no issue and does not mind being held.
