Medical Summary DVM Intake Exam Intact female Estimated age: 5years History : The patient bite a person. Currently on DOH. Subjective: Apparently healthy male intact CAne Corso with alopecia around the neck and dermatitis between the 5th and 4th digit. Observed Behavior – QAR Objective T = Not taken P = 100 bpm R = Not taken BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, alopecia around the neck ( dorsal part), dermatitis inbetween the 5th and 4th digit of the forelimb. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Dermatitis Alopecia Plan DHPP, Bordetella vaccine Pyrantel Microchip Flea treatment Hold for 10 days Prognosis: Good. Temporary waiver due to DOH HOLD
