DVM Intake Exam; Estimated age: 11-13yrs old based on ocular and dental changes. Microchip noted on Intake? scanned negative. History : stray. Subjective / Observed Behavior – anxious, panting, trying to run while being restrained. did not allow oral examination, tried to nip. Objective ; BCS 5/9; EENT: cataracts, bilaterally, ears clean, no nasal or ocular discharge noted; Oral Exam: unable to fully evaluate as P trying to bite and get away. d/c 4/5; p/d 3/5; PLN: No enlargements noted; H/L: grade 4/6 L systolic murmur; NSR, CRT < 2, Lungs clear, eupnic; ABD: Non painful; no masses palpated, tense. U/G: intact male. testicles smooth and symmetrical. MSI: bilateral grade 4 patellar lucaton. Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat and matted along face. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally. Assessment; heart murmur ; dental calculus ; periodontal disease ; cataracts, bilateral. ; patellar luxation ; matted hair. ; Prognosis: good w proper care. Plan: rec the following, once placed: cardiology work-up; dental cleaning /oral care daily ; grooming ; SURGERY: Permanent waiver due to age and heart murmur
