Medical Summary  Estimated age: 1 yo Microchip noted on Intake? No Owner surrender, reported hypertrophic osteodystrophy. Emaciated, hindlimb weakness, wants to be pet, giving kisses, easily handled and carried  T = P = 100 R = 20 BCS 1/9 EENT: Eyes clear, cropped ears clean, no nasal discharge noted Oral Exam: pink mm w/ CRT <2 s, healthy adult teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact,both testicles palpable MSI: Ambulatory x 4, bilateral paraparesis in hind with delayed proprioception, decreased range of motion in hips- pain on extension bilaterally, mild carpus varus, erythma and inflammation of left hindfoot with moist purulent discharge interditally c/w pododermatitis, milder dermatitis right hindfood, no masses noted, overgrown nails not impeding foot pads CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1.orthopedic disease, painful in hindlimbs (suspect HOD) 2. pododermatitis 3. emaciated Plan: 1. x-rays pelvis/hindlimbs 2. Clavamox 125 mg: 1 tablet twice daily for 7 days Prognosis: fair SURGERY: Temporary waiver due to emaciation and orthopedic disease 1027 added rimadyl 50mg/day
