PLN: No enlargements noted
H/L: NSR, HM 3/6 CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G:2 testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalitie Assessment: heart murmur, dental disease, periocular dermatitis. Prognosis:fair. Plan:tab ou bid x 10d echo, dental , neuter in future is preop workup ok. SURGERY: Permanent waiver due to age , heart murmur
