Female intact cachexic dog Depressed, listless T = Not taken P = 120 bpm R = 28 bpm BCS 1/9 5% dehydraed EENT: Severe eye discharge and conjuctivivitis. Matted hair on the eyes. ears clean, no nasal discharge noted Oral Exam: Dental tartar, Mild gingivitis. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin skin, Matted hair. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Dehydrated Cachexia Severe conjuctivitis and eye discharge Matted hair Medical Behavior Status- Green Medical Status- 3 nc Asilomar Status- treatrehab Fast Track- NA Plan Medical treatment is indicated. Clean and shave hairs on eyes. TAB opthtal mic sid OU for 10 days. Bloodwork- anemia, Elevated liver values Vaccinated, microchipped, Pyrantel given, flea treatment applied. Waived for surgery. 150 mg milk thistle sid PO for 20 days. 0.2 ml Pectinic sid PO Prognosis- guarded.
