DVM Intake Exam; Estimated age: 10 years; Microchip noted on Intake? yes; History : stray; Subjective: Observed Behavior – growling, tries to snap, needs to be muzzled; Objective ; P = wnl; R = wnl; BCS 5/9; EENT: Cataracts OU, moderate ceruminous discharge AU, no nasal or ocular discharge noted; Oral Exam: not performed, muzzled, but noted to have severe dental disease when barking; PLN: No enlargements noted; H/L: Grade IV/VI heart murmur, pulses synchronous, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: neutered male, no discharge; MSI: Ambulatory x 4 with stilted hind limb gait. Painful on extension of both hips. skin free of parasites, soft ~ 3cm in diameter SQ mass noted on the cranial ventral aspect of the chest, healthy hair coat; CNS: Mentation appropriate; palpebral + OU, questionable menace ; CP x4; Rectal: normal externally; Assessment; 1) Hip Pain- suspect OA, but cannot r/o other causes such as soft tissue injury, neoplasia 2) Cataracts- patient appears to be blind 3) Heart Murmur- r/o underlying heart disease (MVD) vs less likely physiologic 4) Dental Disease Prognosis: fair Plan: CTM in medical- monitor gait and appetite; Alerted new Hope; Carprofen2.2mg/kg PO BID x 5 days, then re-eval; Unable to get blood on intake- scheduled for CBC/Chem tomorrow to make sure ok to continue carprofen
