I came in the shelter as a STRAY on 09/18/2017 from NY 10457, owner surrender reason stated was STRAY.

09/21/2017 Exam Type RE-EXAM – Medical Rating is 4 C – SEVERE CONDITIONS , Behavior Rating is NONE, Weight 43.0 LBS.
Hx: hypovolemic shock, severe azotemia, severe electrolyte deraingements, and gastric ileus on intake; started on NaCL with supplemental K, baytril, ampicillin, famotidine and cerenia; renal values gradually improved and were WNL on recheck yesterday; residual mild hyponatremia and hypochloremia; noted to be polyuric; chewed out catheter overnight but was replaced; NaCl running at 60 ml/hr; lepto suspect S/O BAR, alert, standing in kennel wagging tail!!! good appetite!!!! mm pk, moist; CRT 2 sec no nasal discharge or sneezing noted OU-clear, no further mucoid discharge eupnic, heart/lungs WNL soft, nonpainful abdomen underweight A Azotemia-resolved, r/o lepto vs severe dehydration due to other causes Electrolyte abnormalities-resolving Hypovolemic shock/dehydration-resolved; r/o leptospirosis vs addisonian crisis vs other P CWCT send out lepto titers, PCR today rec’d ACTH stim test prognosis: good; still do not know underlying cause of severe dehydration/shock but appears to be improving; overall good prognosis if lepto; addison’s also considered which carries a good long term prognosis with steroid replacement therapy

KNOWN HISTORY: None Date of intake 9/18/2017 Unaltered male, stray SAFER ASSESSMENT: Date of assessment 9/20/2017 Summary: Due to his current medical condition/health status, Bucho is not an appropriate candidate for a SAFER assessment at this time. Bucho appears calm, he has allowed all handling in shelter without any issues. INTAKE BEHAVIOR Date of intake 9/18/2017 During intake, Bucho allowed all handling. MEDICAL BEHAVIOR: Date of initial 9/18/2017 During his initial medical exam, Bucho allowed all handling. ENERGY LEVEL: We have no history on Bucho so we cannot be certain of his behavior in a home environment. In the care center, Bucho displays a very low level of energy, though this may be due to current medical condition. RECOMMENDATIONS: New Hope _X_Place with a New Hope partner – Bucho must be placed with a New Hope partner who can meet his needs medically and then assess behavior once he is in a stable environment.
