Medical Summary Sex: Male neutered Scan: Negative Estimated age: 10-13 years old Fleas: Small amount of flea dirt. S: Quiet, wary, allows most handling after a towel scarf is used. Panting, stiff body. O: BAR-H, BCS 5/9, MMs pink and moist EENT: Pigment keratitis OU (worse OD). AD: dried bloody discharge coming from ear canal. Clean nose. Severe dental disease. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Tense, no direct pain on palp M/S/I: Severe medial buttressing and grade 4/4 medial patellar luxation bilaterally. No skin lesions noted. UG: Male neutered Neuro: Head tilt to right A: 1. Severe pigment keratitis 2. Severe dental disease 3. Severe patellar luxation and arthritis in both stifles 4. Right ear discharge and head tilt: R/O otitis externa, polyp, neoplasia, other Short-term prognosis: Poor P: 1. Gave 1.6 ml pyrantel PO and Activyl 14-22 lbs during exam 2. Examine right ear with otoscope after transport to MACC 3. In addition to treatment of ear condition, long-term veterinary care should include management of knee arthritis and eye disease, as well as dental cleaning/extractions. 1088
