Objective – T = – P = wnl R = wnl BCS = 4.5/9 EENT: Suspect Blind – no menace OD – Bupthalmosis vs exopthalmosis, mydriasis, Negative PLR, Negative Menace, corneal graying. Mild Discomfort. OS – Normal ocular size, mydriasis, Negative PLR, Negative PLR, cannot visualize the lens or optic nerve, the tapetum is unobscured and easily visible. Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: moderate tartar on canine PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: INTACT Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: externally normal Assessment: Blind – OD – Bupthalmosis vs exopthalmosis – R/O Glaucoma vs retrobulbar mass – NO TONOMETRY DEVICE AVAILBLE- OS – lens not visible – R/O lens luxation Prognosis FOR VISION – guarded Plan: -OVH and Recommend OD enculeation -Gave 0.15 ml Rimadyl SQ once only on intake -Place on Rimadyl 1/2 of a 25 mg tablet SID for 5 days. SURGERY: Okay for surgery

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth.
Tag: 1. Dog follows at the end of the leash, body soft.
Paw squeeze 1: 2. Dog quickly pulls back.
Paw squeeze 2: 2. Dog quickly pull back.
Toy: 1. Minimal interest in toy, dog may smell or lick, then turns away.
Summary: Bubbles approached the assessor with a soft body. She was social during the assessment, allowed all handling, and displayed no concerning behaviors.
Summary (1): 7/8: Bubbles allows other small dogs to greet her and keeps to herself.
Summary (2): 7/9: Bubbles wanders the yard mostly keeping to herself.
