04/24/2017 Exam Type RE-EXAM – Medical Rating is 5 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is AVERAGE, Weight 67.0 LBS.
04/24/17 Sedated with Dexdomitor 0.8 ml IV. Radiographs of abdomen and thorax obtained: Decreased serosal detail with abdominal distension, difficult to fully asses organs due to ascites but appears to have mid-abdominal mass. Thorax WNL. Palpation while sedated allows for better assessment of mass: large, irregular, mobile mass that appears to be associated with intestines. Moderate effusion that was not as evident yesterday, indicating that it is recent. Need to rule ou abdomina bleeding. Prepped ventral abdomen. Using 3 cc syringe, 21 ga needle, obtained sample of effusion. Fluid is clear, straw colored, no evidence of bleeding. Assessment – Likely malignant neoplasia, possible intestinal in origin. Plan – Poor prognosis. No therapy indicated at this time as pet is comfortable and stable, with no pulmonary involvement. May be a candidate for hospice care, but expect limited survival time.

04/14/17 13:30 Bryan is a six year old tan male American Pitbull terrier. He was given to owner by a neighbor and was with his owner for a little over four years. Bryan was not taken to the vet by his previous owner. Bryan has lived with two adults and two small children ages four and five. Bryan is described as friendly and outgoing towards strangers and will engage in exuberant play with the adults in the home. Bryan is described as gentle and relaxed towards the children and was always aware of his size. Bryan has spent time with small dogs and owner stated he does well and will engage in exuberant play. Bryan will not play with large dogs and will generally sniff and keep his distance. Bryan spent time with a cat and was said to be playful and would cuddle with the cat in the home. Bryan does not have a bite history. Bryan is said to have great behavior in the home according to his owner. Bryan is fed Alpo brand wet and dry food and is fed once a day. Owner also gave Bryan rice mixed with his food as well. Bryan does not mind if his food is touched or approached while eating. If on the furniture Bryan can easily be pushed off. Bryan is fearful when bathed and likes to be brushed. Bryan will bark when a stranger knocks on the door at first then sniffs them once they enter. Bryan is described as a playful, friendly and affectionate dog. Bryan is walked twice a day and is used to leash walks. Bryan loves to play with balls and likes to play fetch and chase. When his owner is home he will follow them around the home. Bryan usually sleeps anywhere. He is well behaved when left alone in the home and was not crate trained. Bryan knows the commands sit, come, and “give me five”. Bryan walks well on leash and stays close to his owner’s side when off leash. He loves biscuit bones.

Behavior toward cats: Playful and cuddles with cats Resource guarding: None reported Bite history: None reported Housetrained: Yes Energy level/descriptors: Bryan displays a high activity level in the household. Other notes: Upon intake Bryan was friendly and allowed all handling. SAFER ASSESSMENT: 4/21/17 Look: 1. Dog holds gaze with soft eyes, soft body. He allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog leans into the Assessor, eyes soft, soft and loose body, open mouth. Tag:1. Follows at end of leash, body soft but a bit distracted. Squeeze 1&2: 1. Dog gently pulls back his paw. Toy: 1. Minimal interest in Toy, dog licks, then turns away. Summary: Bryan was very friendly and affectionate towards the assessor. According to Bryan’s previous guardian, Bryan has spent time with small dogs and owner stated he does well and will engage in exuberant play. Bryan will not play with large dogs and will generally sniff and keep his distance. The Behavior Department recommends that Bryan be placed with dogs that match his calm sociability. 4/20: Bryan displays soft body language when greeting a novel female dog. 04/19/17 DVM exam. History – Brought in by owner who has had him for 4 years, no medical concerns noted. Microchip scan negative. Age 6 yr. Observed behavior – Friendly, social, wagging tail, allows all handling. Evidence of cruelty – No. Evidence of trauma – No. O: BARH. mm=pink, moist, CRT<2 s. HR =80, RR=16, BCS=5/9. ORAL: Mild calculus and gingivitis. EENT: No oculonasal discharge, no sneezing. H/L: Lungs clear, no m/a, no coughing or dyspnea. ABD: Tense, but not painful. No obvious masses/organomegaly. UG: Male. Both testicles palpable. MS: Amb x 4 no lameness. INTEG: Full, clean haircoat. 5 mm raised, flat growth lat R elbow. NEURO: No neurologic deficits. A: Dental disease, small skin growth. Otherwise apparently healthy adult dog. P: OK for neuter surgery. Excellent prognosis.
