DVM Intake Exam: Estimated age: 6-8yrs Microchip noted on Intake? no Microchip Number (If Applicable): History: Stray Subjective: Dog is BARH Observed Behavior – immediately social and friendly despite severe eye problem (most likely has limited to no site), allows all handling with minimal restraint, relaxed body Evidence of Cruelty seen – yes (neglect of eye issue). Evidence of Trauma seen – no. Objective: T = NA P = 120 R = pant BCS 5/9 EENT: Eyes – bilateral exophthalmos, corneas are dry with crusted mucopurulent discharge (STT <5mm/min OU), ears – moderate erythema with yellow waxy discharge and lichenification, no nasal discharge noted. Oral Exam: Grade 1+/4 dental dz. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated. U/G: male intact, both testicles descended. MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair coat unkempt/matted CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: not performed Assessment: Exophthalmos and KCS OU; Otitis externa, AU Prognosis: Good for general health; Poor for vision Plan: Adding LVT ACR to clean and medicate ears. Recommend rescue placement due to severity of ocular issue. Starting artificial tears – thin strip OU q12h (indefinitely). Recommend ophtho consult ASAP on placement. SURGERY: Okay for surgery
