09/25/17 20:10 Basic Information: Brownie is a 13 year old, male, small breed dog. The previous owner got Brownie as a gift from a friend about 12 years ago and is surrendering him because of personal problems. The dog does seem to have to eye problems and was last at the vet about 2 months ago. Socialization: Brownie is friendly with strangers and adults and will play gently with them. He is friendly with children who are 9 years and older. He hides when younger children come over and will not play with them. He will run away from other dogs and has never been around cats. He has never bitten another animal or person. Behavior: Brownie is house trained but recently he has been having pee accidents in the home on a daily basis. He will hide during fireworks and thunderstorms He isnt bothered with having his food, treats, and toys touched. He isnt bothered with being pushed off furniture and being held. He doesn’t mind being disturbed while sleeping and is friendly with people coming towards the home and owner. He does not like having his nails trimmed and will growl and snap. He is good with baths and being brushed. For a new family to know: Brownie is a friendly, affectionate, playful, mellow dog that has a medium activity level. He loves to get walks and will follow you around the home. He doesn’t really care about toys and is mostly indoors. He sleeps in the bed with the owner and eats Cesar’s dry food with an automatic feeder. He loves beggin stripes treats. He is mostly house trained since he has had some accidents on a daily basis. He does prefer the grass and cement when he does go outside. He is well behaved when alone and knows how to sit. He likes slow walks on the leash about 3 times a day. He may pull lightly. He has never been walked off leash. Upon arrival, Brownie had a wiggly body and allowed counselor to pet, collar, and pick up.

Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is closed. Tag: 1. Follows at end of leash, body soft. Squeeze 1: 1. Dog gently pulls back his paw. Squeeze 2: 1. Dog gently pulls back his paw. Toy: 1. Minimal interest, dog sniffs toy. Summary: Browniee displayed no concerning behaviors on his assessment. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: 9/27: When introduced off leash to other dogs, Browniee greets politely and wanders the pen. INTAKE BEHAVIOR Upon intake, Browniee had a wiggly body and allowed handling. MEDICAL BEHAVIOR: 9/25/17 During his initial medical exam, Browniee was docile and allowed all handling. DVM Intake Exam Estimated age: 13yrs Microchip noted on Intake? neg History : o surrender Subjective: Observed Behavior – docile, allows all handling Evidence of Cruelty seen – yes Evidence of Trauma seen – no Objective P = 130 R = 24 BCS 4/9 EENT: nuclear sclerosis OS seen, OD: severely crusted with large amounts of dried d/c causing the closing of the eye partially, when gently opened with sterile saline, surface of cornea has chronic changes- black pigment, cloudy appearance, moderate purulent d/c, ears clean, no nasal discharge noted Oral Exam: moderate tartar and gingivitis PLN: No enlargements noted H/L: NSR, grade 2/6 systolic HM, no arrythmias, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. On surface of anus at approx “7 o’clock“- a red approx 2cm mass U/G: MI, both testicles descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. BCS 4/9 CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NSF, feces in distal colon Assessment MI moderate dental and periodontal dz OU: r/o KCS with active infection OD peri anal anal mass: r/o benign vs. malignant HM: r/o valvular, myocardial, other underweight Plan STT: inconclusive results – OD- the ocular /c got stuck on STT strip so unable to assess results, 13mm/min on OS TAB ointment OD BID x10 optimmune OU BID x10 recheck eyes and STT in 7 days, extend optimmune long term if effectice rec biopsy/histopath of perianal mass, rec ECHO for HM to further assess.
