DVM Intake Exam: Estimated age:8y Microchip noted on Intake?n Microchip Number (If Applicable): History :stray, o known Subjective:large salivary mucocoele submandibular area, confirmed by aspiration of saliva type fluid from mass. wound from embedded collar ventral neck (no collar present)–most likely the inciting cause of the mucocoele Observed Behavior -snaps, impossible to examine without muzzle Evidence of Cruelty seen -y Evidence of Trauma seen -n. Objective: T = P =100 R =wnl. BCS 5/9 EENT: some sclerosis, ears clean, no nasal or ocular discharge noted. Oral Exam: heavy tartar PLN: No enlargements noted H/L: NSR,hm 2/6, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: intact testes x2. penis extruded during exam but seem not to be paraphimosis MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities. Assessment: salivary mucocoele, secondary to embedded collar. heart murmur. Prognosis:good. mucocoeles can be hard to fix without surgery but are not dangerous. recc echo. Plan: seek nh placement after investigation. SURGERY: Temporary waiver due to illness S/O: BAR. Growling, snapping on approach, started lunging at front of cage. Food bowls empty, excellent appetite. Cage exam only. EENT: Severe dental disease – missing teeth, malalignement, severe tartar and gingivitis – nuclear sclerosis, no ocular or nasal discharge HL: No coughing or sneezing, normal RR/RE. MSI: Ambulatory x 4, no obvious lameness, full coat, no mass seen under mandible, only very mild swelling; circumferential wound around neck – deepest at ventral neck with ~1-2cm area of dark (necrotic?) crusts. UG: Male A: Geriatric, dental disease, heart murmur ausculted on intake exam, wound from embedded collar large mucocoele on intake – not apparent today P: Adding clavamox 62.5mg PO q12 x 10 days and rimadyl 12.5mg PO q24 x 3 days. Good prognosis
