Location: Manhattan, Intake Date: 11/16/17, Intake Type: Stray
Medical Behavior: Blue, Sex: Female, Age: 10 years, Weight: 9.4 lbs
DVM Intake Exam; Estimated age: 10-15 years old; Microchip noted on Intake? Scanned neg on intake; History : Police intake, no hx available; Subjective: Alert, wagging tail, jumps around and cuddles into handler; Observed Behavior – Friendly, allows all handling; Objective ; BAR, MMs pink, BCS 2-3/9, severe generalized muscle wasting; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Moderate tartar; PLN: No enlargements noted; H/L: Grade 5/6 systolic heart murmur with palpable thrill. Tachypneic but appears comfortable breathing.; ABD: Severely distended with obvious fluid wave. Soft, non painful, no masses palpated; U/G: Female. 1-2 cm firm irregular tumor associated with the right caudal mammary gland. No other mammary masses noted.; MSI: Stands with elbows and knees adducted. No swelling or crepitus palpated in shoulders or elbows. Mild crepitus and medial patellar luxation palpated in both stifles. Skin free of parasites, healthy hair coat. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Normal externally; Chest and abdominal rads performed; Chest – Extremely large heart, VHS 13.75. Left atrium is visible dilated. Evidence of congestive heart failure – patchy alveolar pattern with air bronchograms over the heart and in the caudodorsal lung fields, in both lateral and VD views.; Abdomen – Loss of serosal detail due to a large fluid/soft tissue opacity in the abdomen; intestines have risen to the center of the abdomen on both lateral and VD views.; Assessment; 1. Severe heart disease: heart murmur, ascites, congestive heart failure, tachypnea, cachexia; 2. Mammary mass; 3. Stifle and suspected elbow arthritis; Prognosis: Guarded for long-term survival. Pt may have a few weeks to months left. Plan: Move to Medical. Treat with Lasix and other meds. SURGERY: Permanent waiver due to heart failure, Hx: Severe heart disease with left atrial enlargement and some degree of congestive heart failure. Pt also has a small mammary mass, and stifle arthritis. Today, pt is energetic, barking in kennel. Resting resp rate is ~50 breaths/min.; Start 0.2 ml furosemide IM q12.
