Medical Behavior Evaluation BLUE
Medical Summary Brought in by owner. History of possible back pain, no medical paperwork submitted. Microchip scan pos. #981020017411764. Quiet, calm, allows all handling. BARH. mm=pink, moist, CRT<2 s. P = 88 R = 24 BCS = 5/9. ORAL: Mod. calculus and gingivitis. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. No coughing. ABD: Non painful, no masses palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lameness. Sensitive on caudal lumbar palpation. INTEG: Full, clean, well groomed haircoat. No skin lesions. CNS: No ataxia, no CP deficits. A: Age 7 yrs. Dental disease. History of back pain with sensitivity on lumbar palpation – Tdx lumbar IVDD. P: Start Metacam 1.5 mg/ml. 0.5 ml PO q 24 hours x 7 days. Cage rest x 7 days. Recheck in 3 days to see how well pain is controlled. Good prognosis with appropriate management. Temporary waiver from neuter surgery due to back problem.

PROFILE:
Brooklyn is a 7 year old male white Maltese and mini poodle mix; he came to his family from a previous owner and he was surrendered because of his medical issues. Brooklyn has no other health issues or injures, and has not recently been seen by the vet.
Socialization:
When Brooklyn is approached by a stranger he is shy for the first few minutes and relaxes later on and becomes friendly. He has been around a 3 year old child and is relaxed and playful with the child. Brooklyn has been around other dogs and is playful when they approach him. Brooklyn has no bite history.
Behavior:
If individual tries to touch Oreo’s food bowl he isn’t bothered and allows it to be removed. During bathing and brushing Brooklyn is slightly tense but will allow individual to wash and brush him. Brooklyn enjoys being held and wants to play when his toys is taking away. When an unfamiliar person or family member comes to the house Brooklyn isn’t bothered and remains uninterested.
For a new family:
Brooklyn was described as playful, affectionate, and friendly with a high activity level by his previous family. Brooklyn enjoys playing with balls and socks. Brooklyn is feed wet food with a little dry food twice a day. He is house trained and uses wee-wee pads or goes outside to use the bathroom twice a day on cement 3 times a day. . Brooklyn has been crate trained, but does not use the crate anymore. While on leash likes to take brisk walks and has never been off leash.
Admission behavior:
When the counselor approached Brooklyn timid his eyes were averted and body was tense. He allowed the counselor to collar and take a photo of him
