5/1/16 : VC – EXCESSIVE DERMATITIS/INFLAMMATION OF SKIN; ESP IN REAR AREA; SOME POSTULES INFECTED IN INNER THIGHS; CLOUDY YELLOW DISCHARGE FROM SOME; HIND AREA RED AND HAIRLESS WITH SORES; SPINE PROTRUDING IN A SLIGHT ARCH BY REAR; OTITIS; LEFT EYE COMPLETELY GRAY. S/O: Dog is QAR, very sweet, allows all handling; BCS 4/9; EENT – ears were cleaned and mometamax applied yesterday; OU – cloudy; STT: OD <10mm/min; OS <5mm/min; Grade 3/4 dental dz; left upper canine and proximal incisor broken; approx 5mm gingival ulceration above pre-molar upper left side; Generalized pyoderma/dermatitis consisting of partial alopecia, erythema, pustules, and crusts; A: Geriatric underlying – seborrheic dermatits, KCS, dental dz; P: Recommend rescue placement only; this dog will need long-term management for chronic ear, eye, and skin issues; Start Optimmune OU BID; Put on for tech check to shave and give chlorhexidine bath tomorrow. Continue Mometamax SID x 5d; Start Clavamox 125mg – 1 tab PO q12h x 10d (gave 1st dose this evening). PROGNOSIS: Fair-Poor
