Medical Behavior Evaluation BLUE
Medical Summary scan neg ~8months, female S/O: QARH. Soft muzzle placed for exam. Nervous, allowed all handling. BCS 5/9 EENT: Teeth clean on visual oral exam only, eyes clear, no ocular or nasal discharge, ears wnl HL: Normal thoracic auscultation, lungs clear, no murmurs/arrhythmias ABD: Normal abdominal palpation, soft, non-tender MS: Ambulatory x 4, no apparent lameness INTEG: Multiple old healed wounds on face/head and distal limbs; ~4cm deep irregular laceration at lateral/caudal right elbow with granulation tissue forming around edges and deep to wound UG: Female, no spay scar seen or palpation A: Multiple old healed wounds; open wound at right elbow – wound does not appear fresh (occured several+ days ago), possible bite wound that healed improperly vs foreign object (fencing vs other) P: Rec clavamox 375mg po q12 x 10 days and rimadyl 50mg po q12 x 5 days. Scheduled for sedated wound debridement and repair tomorrow. Excellent prognosis
