Medical Summary DVM Intake Exam Estimated age: 10+ (dog came to ACC on 12/8/07 and received a microchip only) Microchip noted on Intake? Yes- 0A10535F55 History : Stray but had collar. person bringing dog in was able to put on leash and had no problems. Subjective: BARH. Observed Behavior – Dog appeared at ease but watching our actions closely. Dog fine when petting head and checking ears and looking at eyes but bared teeth when moving near mouth on top of muzzle, then tried to bite when re-approached muzzle. When trying to muzzle, dog growled, bared teeth and lunged at muzzle. Once muzzled, still growled during exam intermittently. Difficult to restrain for HWT – strong dog. Evidence of Cruelty seen – none. Evidence of Trauma seen – none. Objective BCS= 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted. Oral Exam: Mod tartar but tried to bite when checking. PLN: No enlargements noted H/L: NSR, NMA, MM pk. Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: No spay scar seen but unable to check well due to behavior and skin folds/ thick hair on abd. No MGT’s. No vulvar d/c. MSI: Ambulatory x 4, skin free of parasites, healthy hair coat, white around muzzle. LFL – 2 cm soft dermal lichenified and hyperkeratotic cyst lateral antebrachium approx 4 cm below elbow. RFL – 3 cm firm dermal lichenified and hyperkeratotic cyst R lateral elbow. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done. Assessment: 10 yo F Rott – unsure if intact – scan positive. Plan: All vx and HWT done. Prognosis: Excellent. SURGERY: Permanent waiver due to geriatric age.
