Medical Summary DVM Intake Exam Estimated age: 3-5y Microchip noted on Intake? n History : stray Subjective: Observed Behavior – allowed full exam, docile Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 92 R = wnl BCS 4/9 EENT: Eyes clear, mild ceruminous debris AU, no nasal discharge noted Oral Exam: malocclusion with mobile mandibular incisors (maligned) PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: suspected intact female with large (7-8cm diameter) mammary mass / mastitis of 1st mammary gland right side, small 1cm diameter mass cranial to large mass MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: large 7-8cm mammary mass/area of mastitis 1st mammary gland right side w/ smaller mass cranial to it- ddx benign vs malignant; dental dz Plan: rec spay with sx excision of mammary masses w/ biopsy Prognosis: fair SURGERY: Permanent waiver due to possible mammary gland tumor
