Medical Summary Estimated age:9yr Microchip noted on Intake? scan positive. S/O: brought in with suspected mass on back that was bleeding after bx’ed at rDVM, came in with bandage on back, results unavailable. Gentle, allowed all handling, a little nervous when performing exam. P = 120 R = pant but eupneic BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: moderate tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: slightly tense but Non painful, no masses palpated U/G: MC, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, bandage on mid thoracic dorsum with tape going around circumference of abdomen present, bloody discharge on bandage. Removed, and tried to clip fur, approx 5cm firm raised SQ mass with ulcerations and small amount of dried pus on surface, fur has dried blood on it surrounding mass, unable to perform full ortho exam CNS: mentation appropriate – no signs of neurologic abnormalities urinated and defecated after exam Assessment overweight moderate dental and periodontal dz mass on dorsum: r/o benign vs. malignant, as per prev O previous DVM tested, no results available Plan con’t Amoxiclav 500mg tabs ( 1 tab PO BID x 7 days) rimadyl 75mg: 1T PO BID x2 for discomfort associated with ulcerated mass rec mass removal and submit for bx- given that mass is ulcerated, would be best. Consider chest 3v met check prior to surgery, also rec cbc/chem/t4 long term rec weight loss Prognosis: open depending on nature of mass SURGERY: n/a, already nuetered
